Where a filled-in form goes and what the person sees afterwards.

Give the button inside the form the Submit form action. It validates the fields — a required one that is empty stops it — and sends the submission.
A submission lands in two places at once: the Submissions section in the editor (the archive) and a Telegram message to you — in the chat with the bot where you signed up. The second can be switched off with “Send to me in Telegram”.
“Message after sending” is what the person sees. It is usually followed by Navigate to screen with a thank-you page.
Add to the request carries what the person picked rather than typed: a master, a date, a time, a seat category. None of that is a form field, so it is named here: the label you will see on the left, the value on the right — {{slotTime}}, say. Without it the request arrives half empty and you end up calling to ask.
